This is a description of a prescription strength toothpaste called PreviDent®5000 BoosterPlus, containing 1.1% sodium fluoride. It is indicated for once daily self-topical use and is established to be safe and effective as a caries preventive. It is not recommended for systemic treatment and is not to be swallowed. The toothpaste should be used once daily in place of regular toothpaste. The dosage instructions for adults and pediatric patients are given. The toothpaste is contraindicated for pediatric patients under age 6 years, and caution should be exercised when used by nursing mothers. The product is supplied in 3.4 FL OZ (100 mL) plastic bottles and stored at controlled room temperature 68-77°F (20-25°C).*
