For patients with a clearance rate of 15 - 29 mL/min/1.73 m2, the recommendation is to an initial dose of 200mg followed by 100 mg administered on alternatingdays. Finally, for patients with a clearance rate of less than 15 mL/min/1.73 m2, the recommended dose is 200mg every 7 days.*
