The 0.75% concentration of Sensorcaine injection is not recommended for obstetrical anesthesia due to reports of cardiac arrest and death during use of Bupivacaine for epidural anesthesia in obstetrical patients. The 0.75% concentration should be reserved for surgical procedures where a high degree of muscle relaxation and prolonged effect are necessary. Local anesthetics should only be employed by well-versed clinicians who have the necessary treatment resources including cardiopulmonary resuscitative equipment, and the correct personnel to manage any acute emergencies. Mixing or the prior or intercurrent use of any local anesthetic with Bupivacaine is not recommended due to insufficient data on the clinical use of such mixtures. Bupivacaine with Epinephrine 1:200,000 should not be used concomitantly with ergot-type oxytocic drugs, and solutions of Bupivacaine containing a vasoconstrictor, such as epinephrine, should be used with extreme caution in patients receiving monoamine oxidase inhibitors. Lastly, Bupivacaine is not recommended for use in intravenous regional anesthesia (Bier Block) because there have been reports of cardiac arrest and death during its use.*
