The usual adult dosage is 2 g in divided doses. Each 10 mL of the resulting solution contains 500 mg of vancomycin and each 20 mL of the resulting solution contains 1g of vancomycin. The bottle must be discarded within 4 hours of initial entry, and the container closure is not made with natural rubber latex.
