Cyclophosphamide is a hazardous drug that is available in a multiple-dose vial. The vial contains 1,000 mg of cyclophosphamide in 5 mL solution, with a concentration of 200 mg/mL. Before administering it intravenously, it must be diluted. Each mL of the solution contains 214 mg of cyclophosphamide monohydrate, which is equivalent to 200 mg of cyclophosphamide USP. The vial should be stored refrigerated at a temperature between 2° and 8°C (36° to 46°F). The lot number is Y0000 and the barcode is YYYY-MM.*
